Production Compensation - 33% of revenue produced over $1 million.
Travel Reimbursement - for surgeons not on Long Island - $200/day.
Surgeon Referral Bonus – 20K for any surgeon referred and we hire (payable after 1 year).
Benefits – reimbursement of 100% of current health, dental and eye plans. 401K with 3% ASC contribution (even if they don’t participate themselves).
Business Expenses – 5K for licenses, professional associations and CE.
Paid Days Off – 20 days for vacation, sick and personal time (PTO). 5 days to attend CE.
Work Week – 4 days/week.
Profit -Sharing - XXX% of net income (TBD) of the office(s) the new surgeon works in.
Equity/Shares of Stock - good faith representation that after 2 years Dr. Infernuso will consider providing equity/shares of stock in the P.C. if the relationship has evolved to the satisfaction of both parties.
